◆ $0 pump claim based on pump cost data for Tandem customers with pharmacy benefits (3/2/2026 - 7/31/2026). $0 pump supplies claim based on supply cost data for Tandem customers with pharmacy benefits and use of the Tandem copay card (3/2 /2026 -7/31/2026). Pump supplies cost calculation reflects the average monthly out- of-pocket cost for disposable infusion sets and cartridges necessary to utilize Tandem pump therapy and assumes consumption of 10 infusion sets and cartridges per month. The Tandem copay card has a maximum benefit of $100 per month for 12 months. Copays less than $100 will result in complete coverage for disposable infusion sets and cartridges only. The Tandem copay card is not available for customers on Kaiser, Medicare, Medicaid, or TRICARE plans. Additional terms and conditions apply.
